Keith Gallus, Pharm.D.

Director of Pharmacy

Dr. Keith Gallus earned his Pharm.D. degree from the University of Minnesota College of Pharmacy in 2003.  During pharmacy school, he was involved in the Kappa Psi Pharmaceutical Fraternity and was a recipient of the Eli Lilly Achievement Award.  In addition, Keith has a bachelor’s degree in Chemistry from St. John’s University. 

After pharmacy school, Keith worked for several years at an independent pharmacy in his home state of Minnesota.  During this time, he became involved in the American College of Veterinary Pharmacists, and was certified in immunization delivery. 

In 2007, Keith and his wife relocated to Arizona.  As Keith had worked many years in an independent pharmacy, he was looking for a job in Arizona that allowed him to interact more with his patients and keep him in that community pharmacy setting. 

Since becoming a pharmacist with The Apothecary Shops in October of 2007, Keith has had specialized training in veterinary care and compounding.

Carolyn S. Wright, Pharm. D.
Director of Pharmacy

Dr. Carolyn Wright earned her pharmacy degree in 2007 from the University of Tennessee Health Science Center where she graduated Cum Laude. She also holds a bachelor of science in Chemical Engineering. During pharmacy school, Carolyn was active in Phi Delta Chi fraternity.

In March 2009, following internships and pharmacy positions in retail and Veterans Affairs, Carolyn joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada, California, Utah and Ohio.

A licensed pharmacist in Arizona, Carolyn is also certified in immunizations and CPR. Her areas of expertise include veterinary, hormone therapy, compounding and pain management.

Carolyn lives in Gilbert, AZ and enjoys cooking.

Dan Lindell, Pharm.D.

Director of Pharmacy

Dr. Dan Lindell joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, in 1999 as a compounding technician. In 2004, he earned his doctoral degree from Midwestern University College of Pharmacy in Glendale, Arizona, graduating summa cum laude.
That same year, he was put in charge of the 15th Avenue pharmacy in Phoenix.

Dan specializes in Infectious Diseases, transplant treatments and general pharmaceutical compounding.

Dan is a graduate of the University of California – San Diego, where he studied biology.

He and his wife Jennifer live in Phoenix and enjoy playing golf, mountain biking and reading.

Jackie Cavanagh, Pharm. D.

Dr. Jackie Cavanagh earned her pharmacy degree from the University of Arizona, College of Pharmacy in 2005. During pharmacy school, Jackie was involved in several pharmacy related fraternities and industry organizations including AzSHP, NCPA, ASP and Operation Immunization. In addition, Jackie holds a bachelor of science in microbiology from Arizona State University.

Following a pharmacy internship at Tucson Medical Center, she completed her pharmacy practice residencies at the Southern Arizona VA Medical Center in Tucson, Good Samaritan Hospital in Phoenix, and the ASU Outpatient pharmacy in Tempe.

Jackie joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada, Utah and Ohio in June 2006. Prior to joining the Tempe store, Jackie served as pharmacy director at our Phoenix III location.

Jackie’s pharmacy background includes retail, hospital and compounding experience. A licensed pharmacist in Utah, Arizona, Ohio and Nevada, Jackie also is certified in sterile compounding through The American College of Apothecaries (ACA). Her areas of experience include fertility services, hormone therapy, compounding, pain management and HIV/AIDS.

Dr. Justin Brock earned his pharmacy degree in 2008 from Arizona’s Midwestern University, College of Pharmacy.  During pharmacy school, Justin was involved in several fraternities and industry organizations including National Community Pharmacy Association (NCPA), the American Society of Health System Pharmacists (ASHSP), American Pharmacists Association—Academy of Student Pharmacists (APhA-ASP), Student Council, Phi Lambda Sigma Leadership fraternity, and Phi Delta Chi Professional Pharmacy fraternity.  While in college, he also earned several honors and awards.

In July 2008, following internships in hospital and retail, Justin joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada, California, Utah and Ohio.  Prior to joining the Scottsdale/91st St. store, Justin served as pharmacy director at our Gilbert location.

Justin’s pharmacy background includes retail, hospital and compounding experience. A licensed pharmacist in Arizona and Ohio, Justin is also certified in immunizations, CPR and is a Tobacco Cessation Brief Skills instructor. His areas of expertise include pain management, veterinary medicine and hormone therapy.

Justin and his family live in Gilbert, AZ. He enjoys sports, reading, movies and spending time with family.

Linda Cappellini, R.Ph., FACA, FIACP

Women’s Health Consultant

Linda Cappellini received her pharmacy degree from Temple University School of Pharmacy in Philadelphia where she was a member of The Rho Chi National Honor Society.

In 1996, Linda joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. Her dedication to the profession is evident with several certifications in compounding, including women’s health, aseptic and sterile products, and pain management.

She currently supervises the Compounding Quality Assurance program for the entire Apothecary Shop organization, which is responsible for more than 6,000 compounded prescriptions per month. She also oversees the company's Women's Health Program.

Linda is an Adjunct Clinical Preceptor for University of Arizona College of Pharmacy in Tucson, Ariz., and Midwestern University College of Pharmacy in Glendale, Ariz., and in 2002 was honored as Preceptor of the Year. She is a full fellow in the American College of Apothecaries (ACA) and is a member of the International Academy of Compounding Pharmacists (IACP), the American Pharmacists Association (APhA) and the North American Menopause Society (NAMS). She also is NIPCO certified in Osteoporosis Care.

During the last five years, Linda has focused her expertise on Women’s Health Care. She has completed numerous courses and has received various certifications on the subject. Linda is the only pharmacist in Arizona who has achieved both Certified Menopause Educator and Certified Menopause Practitioner degrees.

Susan G. Pasco, R.Ph.

Director of Pharmacy

Susan Pasco earned her pharmacy degree from University of Illinois Chicago College of Pharmacy. She comes from a family of three generations of pharmacists and has 27 years of pharmacy experience. She has completed extensive course work in the area of hormone therapy, compounding and counseling.

After graduation, Susan evaluated the many opportunities available in the pharmacy field. She has experience working in hospitals, retail, specialty, HMO and compounding pharmacies, and also has held supervisory positions throughout her career.

In 2003, Susan joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. During the last 10 years of her career, Susan has focused her pharmacy experience on compounding, women’s health, oncology, fertility and veterinary medicine. As a breast cancer survivor, Susan is able to share, comfort and touch the lives of her oncology patients by drawing on her personal experiences and firsthand knowledge.

Kim McCroden, Pharm.D.

Director of Pharmacy

Dr. Kim McCroden earned her Pharm.D. degree from Creighton University School of Pharmacy in 2006. During pharmacy school she was actively involved in student government, the Kappa Psi Pharmaceutical Fraternity, the American Society of Health System Pharmacists, and the National Community Pharmacists Association.

After pharmacy school, Kim relocated to Arizona from her home state of Nebraska, and joined the Apothecary Shop staff. Having worked many years in a retail pharmacy chain Kim was looking for a job that allowed her to focus more on patient care, and have the ability to specialize in different areas. Since becoming a pharmacist with the Apothecary Shops, Kim has become certified in immunization delivery, and has had specialized training in diabetes management, veterinary care, and compounding.

Kim and her husband live in Chandler and enjoy traveling and rooting for their home state Nebraska football team.

Eric S. Sredzinski, Pharm.D.

Director of Pharmacy

Dr. Eric Sredzinski graduated with a Doctor of Pharmacy degree from the University of Arizona College of Pharmacy in Tucson in 2000. As a student, he twice received national recognition by the American Society of Health-System Pharmacists (ASHP) and earned a Rho Chi certificate of merit. Following a pharmacy internship at Tucson Medical Center, he completed his pharmacy practice residency at the Southern Arizona VA Medical Center in Tucson, where he served as a clinical weekend pharmacist and IV room pharmacist.

In November 2001, Eric left his position as a staff pharmacist at University Medical Center in Tucson to join The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. His areas of expertise include oncology, pain management and infectious disease.

In addition to being a member of the adjunct faculty at University Medical Center, where he teaches fourth-year pharmacy students, Eric is a member of the ASHP and American Pharmacists Association. His research experience has included studying the management of anticoagulated patients and trends in antidepressants, and he has presented to professional audiences on a number of health topics.

Eric is also active in community education, and has frequently presented to the Arizona Deaf and Hard of Hearing Society. In February 2005, he launched a series of transplant, pain management and Hepatitis C education classes at the Tucson Apothecary Shop location. Individualized personal health consultations also are available by appointment.

Crystal Schneider, Pharm. D.

Dr. Crystal Schneider earned her Doctor of Pharmacy degree from the University of Arizona College of Pharmacy in Tucson, Ariz., in 1999. As a student, she served as Kappa Epsilon Pharmacy Fraternity President, was a member of The Rho Chi National Honor Society and received The American Pharmacists Association-Academy of Student Pharmacists (APhA-ASP) Professionalism Award. She currently serves as an extern preceptor (teaching students in their final year of training) at the University of Arizona.

After graduation, Crystal completed her pharmacy practice residency at the Seattle, Washington VA Medical Center where she served as a clinical pharmacist and IV room pharmacist. In 2000, Crystal moved back to Tucson where she joined the pharmacy staff at the Tucson VA Medical Center. There she served as a pharmacist in the ambulatory care/telephone link care unit.

Crystal joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, in October 2005. Prior to joining the company, she was in a retail setting as a pharmacist in Tucson. Crystal’s area of expertise includes pain management, pediatrics and women’s health.

David Brandebura, R.Ph.

Director of Pharmacy

David Brandebura earned his pharmacy degree from University of Pittsburgh where he served as the Vice President of the Phi Delta Chi Fraternity. He has more than 17 years of pharmacy experience.

In September 2006, David joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, and opened the company’s second out-of-state pharmacy. Prior to joining the company, he was a pharmacist at New Albany Surgical Hospital.

David’s areas of expertise include ophthalmology, women’s health and diabetes. Through the American College of Apothecaries (ACA), he is certified in compounding and aseptic compounding techniques. David is a registered pharmacist in Ohio, Nebraska, Pennsylvania, Louisiana, Tennessee and Arkansas.

Jeb F. Sydejko, Pharm. D.

Dr. Jeb F. Sydejko graduated with a Doctor of Pharmacy degree from University of Southern California in Los Angeles, CA and has more than 20 years experience.  While in pharmacy school, he received various service awards. Jeb also holds a Juris Doctorate from Whittier Law School in Los Angeles, CA.

In November 2008, Jeb left his position as Chief Pharmacist at Sayre Medical Pharmacy to join The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada, Utah, California and Ohio. His areas of expertise include general and independent pharmacy and retail. At our Vegas location, Jeb specializes in vet medicine, fertility, transplant and hormone therapy.

Jeb’s professional experience includes pharmacy consulting to The Help Group in California as well as the California State Board of Pharmacy. Additionally, he is an expert witness in criminal, civil and administrative law cases specializing in pharmacy disciplinary issues, drug information, pharmacy liability and risk management assessment, drug interactions and adverse drug effects.

Jeb is active in community education and has presented to various health-care providers on topics such as writing prescriptions to help minimize errors. He also has authored “Four Steps to a Safer Prescription” and continues to expand his expertise into other areas of health care.

Licensed in California and Nevada, Jeb lives in Las Vegas and enjoys health and fitness, hockey, spending time with family and friends, computers and reading.

Dr. Todd Bleak earned his pharmacy degree from the University of Arizona, College of Pharmacy in 1992.  Upon graduation, he moved to Philadelphia and served as the Community Pharmacy Practice Resident at the University of Pennsylvania Hospital. His post-graduate education includes a Master of Business Administration from the University of Nevada at Las Vegas.

Todd’s pharmacy background includes retail, hospital, compounding, prior authorization roles and managerial experience.  He also has served in several executive-level positions at various entities including Sierra Health Services, Sunrise Hospital and Medical Center and McKesson Medication Management.

Raquel Dina, Pharm. D.

Dr. Raquel Dina earned her doctor of pharmacy degree in 2000 from Western University of Health Sciences, Pomona, CA. During pharmacy school, Raquel was involved in the American Society of Health System Pharmacists, and received the Rich Kane Private Practice Scholarship.

In December 2008, Raquel joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada, California, Utah and Ohio. Prior to this, she was a Pharmacy Manager at AIDS Healthcare Foundation.

A licensed pharmacist in California and New Jersey, Raquel is also certified in administering immunizations. Her areas of expertise include HIV/AIDS, retail and general pharmacy, and she frequently lectures to HIV/AIDS support groups.

Mariya F. Farooqi, Pharm.D.

Dr. Mariya F. Farooqi joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, California, Nevada, New Mexico, Ohio and Utah in 2009. In 2007, she earned her Doctor of Pharmacy degree from the University of Rhode Island.

Mariya specializes in pain management, infectious diseases and general pharmaceutical compounding.

She has also completed a two-year Clinical Toxicology fellowship at the New Mexico Poison and Drug Information Center and is currently working on a board of certification in applied toxicology.

Mariya is a graduate of Oxford Brookes University – England, where she studied nutrition and food science.

She enjoys doing yoga and spending leisure time with her son, mentoring young girls groups and traveling.
